It's simple, just follow these two steps:

Step One - Compile Documentation
In order to complete your 401(k) Review, we request the following information: 

1. Copy of your most recent quarterly statement including:
   Fund breakdowns for your total plan
   Fund performance
   Participant balances by fund allocation

2. Investment Policy Statement, if available

3. Investment committee meeting minutes

4. All administration, recordkeeping, advisory, or investment services agreements

5. Most recent ADP test

Step Two, Submit Information
